⚕️ Legend's Hockey School Medical Liability and Image Waiver

Consent and Assumption of Risk

I/We, the parent(s)/guardian of the above-named player, grant permission for him/her to participate in activities with Legend's Hockey School.

I/We acknowledge that hockey is a contact sport and that participation inherently involves a risk of injury. It is my/our responsibility to ensure my/our son/daughter is outfitted with proper and adequate protective equipment.

I/We confirm that my/our son/daughter is physically fit for all program activities. I/We understand that participation is purely voluntary.

I/We agree to assume all risks and hazards related to participation, including but not limited to games, practices, scrimmages, and transportation to and from the facility related to Legend's Hockey School.

Release of Liability

I/We, the undersigned, hereby waive, release, hold harmless, and indemnify Legend's Hockey School, and any and all coaches, organizers, and trainers, from any claim, including those arising from negligence, for any personal injury, property damage, and/or wrongful death.

Medical Authorization

I/We authorize Legend's Hockey School, its staff (coaching, organizational), and medical, hospital, or emergency personnel to carry out and/or administer all necessary treatment and diagnosis. This includes rendering emergency care when obtaining additional consent is impractical or impossible.

I/We affirm that we either have appropriate medical insurance or, in its absence, agree to pay all costs of emergency and/or medical services incurred on my/our behalf.

This authorization remains in effect unless it is withdrawn by written notice.

Model Release

By participating in Legend's Hockey School, I hereby grant Legend's Hockey School the irrevocable right and permission to film, photograph, and record players voice and likeness. I agree that Legend's Hockey School owns all rights to these images and recordings and may use, edit, and publish them in any media format—including but not limited to social media, websites, and promotional materials—without further compensation or approval. I waive any right to inspect the finished product and release Legend's Hockey School from any claims or liability related to the use of my likeness.
